An investigation by the US coast guard says the Titan submersible disaster could have been prevented.
It imploded during a dive to the wreck of the Titanic in 2023, killing everyone on board. The report blamed the operator, OceanGate for the tragedy, saying it failed to follow standard safety protocols.
